Now on the ___ day of __________, 2004, there comes on before the Administrative Judge the matter of the appointment of [name] of [business name, address] as a special process server for calendar year 2005, pursuant to the provision of K.S.A. 60-303. After due consideration thereof, the Court finds that [name] should be appointed generally for calendar year 2005 as a special process server to serve all process anywhere in and out of the State of Kansas and to be allowed the fees prescribed in K.S.A. 28-110 and amendments thereto.
