In order for a private detective agency license to be granted, each officer, director, partner or associate must qualify, regardless of whether those persons intend to actually engage in detective business. Thus each officer, director, partner or associate of the agency must submit a completed private detective application form.
In completing this application, please bear in mind that any false information submitted in this form or any accompanying documents, or falsification of the fingerprints or photographs, constitutes grounds for denial of the application, and may subject you to criminal prosecution. The truthfulness and correctness of all information offered in this application must be verified by signing the application before a Notary Public.

Upon approval of this application, the license and (except for agencies) a pocket identification card will be mailed to out. The license will be valid until December 31 of the year following this year, and will then be renewable every two years. A renewal application will be mailed to you in the Fall of the expiration year.
